ConnectGroups’ Community Link Booth at Fiona Stanley Hospital connects connects patients to non-clinical support that will enable them to manage their conditions following discharge.

The first of its kind, the booth is integral to individuals, families, and carers who require emotional and physical support upon departure from the hospital premises. Patients and those who support them can visit the booth to access information and resources about their condition, and locate local Support Groups and services aligned with their health and mental health needs.

Supported by Lotterywest, the booth is a collaborative initiative by ConnectGroups Support Groups Association WA and the Health Consumers’ Council of WA, and Fiona Stanley Hospital.

The Community Link Booth is situated at the main entrance of Fiona Stanley Hospital.  It is a volunteer program comprising of two teams, one at the hospital and the other providing referrals at ConnectGroups. Patients can engage with the service either at the booth or within the wards.

 

Who does it benefit?

Patients

Linking patients to support outside of the hospital where they can receive social and emotional support, and learn self-management skills.

Family and Carers

Linking family and carers to groups and services that will provide them with emotional support and information.

Clinical Staff

Supporting and complementing the work of clinical staff to assist patients engage with community services post their acute admission.
