ConnectGroups’ Community Link Booth at Fiona Stanley Hospital connects patients at the discharge point to community support services that will enable them to manage their conditions and avoid re-hospitalisation.

The first of its kind, the booth is integral to individuals, families, and carers who require emotional and physical support upon departure from the hospital premises. Patients and those who support them can visit the booth to access information and resources about their condition, and locate local Support Groups and services aligned with their health and mental health needs.

Supported by Lotterywest, the booth is a collaborative initiative by ConnectGroups Support Groups Association WA and the Health Consumers’ Council of WA, and a result of an identified need to reduce the high rate of preventable hospitalisations in Western Australia.

The Community Link Booth launches towards the end of 2018 and will be situated at the main entrance of Fiona Stanley Hospital.
