An education tool designed to support people living with type 2 diabetes. It is delivered by specially trained DESMOND Educators who assist with self-management by increasing knowledge and understanding of what type 2 diabetes can mean.
For people with Type 2 Diabetes. Provides participants with a one day Desmond education session, an initial assessment and follow up with a Diabetes educator, lifestyle educator and/or dietitian through the Metropolitan Healthy Lifestyle Program.
Social and educational group for individuals with diabetes, family members and carers to share experiences and meet new people.
Supports people with Type 2 Diabetes; encourages participants to share experiences and information and support one another.
Works to minimise the impact of diabetes by providing advice, advocacy, clinical services, group meetings, and a range of products and services which make living with diabetes easier.
This service aims to support people with the chronic conditions of diabetes, long term lung disease, heart disease and heart failure
The Program provides care and support for clients at risk of, or diagnosed with chronic conditions such as cardiovascular disease, Type 2 diabetes, kidney disease, respiratory disease and chronic musculoskeletal conditions
A program that aims to reduce the risk of Diabetes and Coronary Heart Disease and improve management of those with Diabetes, Coronary Heart Disease and Chronic Kidney Disease. Provides participants with an individual assessment, education and review by a Diabetes educator and/or lifestyle educator.
A program that supports GPs in Diabetes care. Provides clients with skills and knowledge on topics such as self-blood glucose monitoring and HbA1c, healthy eating, active lifestyle changes, medication and communicating with their health care team.
Perth Diabetes Care is a patient centred multidisciplinary clinic providing services for people living with all types of diabetes and chronic conditions. Our health professionals use current best practice guidelines and state of the art technology to provide individualised health care and education. Our aim is to support the individual to achieve their health goals.

We also operate a number of mobile clinics across the Perth Metropolitan area as well as a teens and young adults Type 1 clinic located at the Telethon Type 1 Diabetes Family Centre.
For young adults aged between 16-35 living with diabetes. Meets bi-monthly.
Provides group education and ongoing support by a lifestyle educator through the Metropolitan Healthy Lifestyle Program.
The UWA Exercise and Performance Centre runs a number of tailored programs aimed at improving the improve health, function and quality of life of patients with a range of chronic and complex diseases. The exercise interventions provided are tailored to help patients meet their current goals, and manage their condition/s.
The Exercise and Performance Centre also supports the clinical training of students undertaking the Masters of Clinical Exercise Physiology, and where appropriate exercise services are provided by postgraduate students under the supervision of Accredited Exercise Physiologists (AEP) and allied health professionals.

Healthy Hearts – Cardiovascular rehabilitation program designed and facilitated by exercise physiologists with the aims of:
• Changing physical activity behaviours
• Preventing subsequent cardiac events
• Improving activity tolerance, muscle atrophy and circulation
• Improving quality of life, and
• Significantly reducing risk factors for comorbidities
Mediswim – Aqua based exercise rehabilitation incorporating strength, mobility and swimming to target metabolic disease or its risk factors. Exercise sessions are individualized and heart rate monitored. Benefits include:
• Improved insulin sensitivity and blood glucose management
• Reduction in weight and blood pressure
• Reduction in medication dependence
CanMoves – Exercise rehabilitation for patients at any stage of Cancer treatment. Aim to provide support, and through use of exercise as adjuvant therapy throughout the treatment process. Benefits include:
• Reduced risk of cancer recurrence
• Improved quality of life
• Improved management of treatment related side effects
